I passed your question along to a friend of mine who is (IIRC) running a Cisco ATA (Vonage) behind his firewall. Hopefully he'll have a better answer. When I got my Vonage service (with the newer Motorola hardware) they included instructions for running behind a Linksys router. It amounted to forwarding the following UDP ports from your firewall to the VoIP box: 53, 69, 123, 5060, 5061, 10000-20000. I know you can safely eliminate 53 and 123 from the list, but I'm not sure about the others.
